Mandibular Molar RCT + Rotary Files
Quick Tips for Rotary File Success in Endodontics
1. Never spin before scouting
Always use a size 10 or 15 hand file first. You need to confirm patency and create a smooth glide path before introducing rotary instruments. Skipping this step often leads to ledging or instrument fracture.
2. Use pre-bent hand files to scout curved canals
Pre-bending a stainless steel hand file with cotton pliers helps it follow severe curvature—especially in MB roots or tight mesial canals. Once a smooth path is established, your rotary file is more likely to follow safely.
3. Consider mechanical glide path files
Using a mechanical glide path instrument—like ProGlider, PathFile, or WaveOne Gold Glider—can ease the transition to shaping files by reducing torque stress and keeping your shaping centered and efficient.
4. Don’t overuse your files
Rotary files experience cyclic fatigue quickly in curved canals. Even if they haven’t fractured yet, repeated use increases risk. Most molars justify single-use instruments to minimize the chance of separation or distortion but we do talk about a system in our course to identify how often files have been used.Â
5. Respect resistance
If your file stalls, don’t force it. Apply a light brush-stroke technique instead—moving coronally rather than pushing deeper. This helps enlarge the canal gradually without creating taper lock or transportation.
6. Locate all canals before shaping
Rotary files will not discover anatomy for you. Make sure to identify MB2s, isthmuses, and lateral canals before shaping begins. Sodium hypochlorite and ultrasonics can help expose hidden entries before you commit to shaping.
7. Confirm and refine working length
Your rotary file might not reach the apex on the first pass—and that’s fine. Shape conservatively, recheck with a hand file, irrigate, and return with the rotary. The goal is control, not speed.
